You may navigate to the Wisconsin Court System - Circuit Court Access website to review forms available for filing.   Additionally, the Wisconsin Court System has an online forms assistant that you may wish to utilize (NOTE: the Civil Forms Assistant is located towards the middle of the page).  

General information about the name change process is contained in Wisconsin Statutes 786.36 and 786.37.  There is a $164.50 filing fee to file a Name Change action.  Forms for a name change, as well as Basic Steps to Handling a Name Change and Basic Steps to Handling a Confidential Name Change can be found on the State of Wisconsin website.
